The consultation request form collects: first name, last name, date of birth, email address, phone number, pronouns (optional), reason for the request, preferred type of meeting, general reasons, a brief description of the request, general availability, preferred means of communication, and preferred language of communication (French / English / no preference). When the request concerns a couple or a teenager aged 14 or older, basic information about the other person concerned (first name, last name, date of birth, email, phone number and pronouns) is also collected.

Purpose

This information is used solely to follow up on the request and discuss possible meeting arrangements.

Hosting and transfer

The site is hosted on Lovable Cloud infrastructure. Data submitted through the form is stored in a secure database located in Canada. No data is sold or transmitted to third parties for commercial purposes.

Third-party providers and services

Only the providers necessary for the site's operation are used: website and database hosting (Lovable Cloud), and the email notification delivery service associated with the domain notify.rlsexologie.com. These providers process information solely to deliver the requested service. No advertising tool or analytics platform is used.

Security measures

The site is delivered over an encrypted connection (HTTPS). Information submitted through the form is validated before being saved and stored in a database protected by strict access rules. Viewing it requires authentication to the administrative area, which is accessible only to Rachèle Labbé. Submitted information is never publicly displayed on the site.

Withdrawal of consent

You may withdraw your consent at any time and request the deletion of information submitted through the form, subject to the retention obligations applicable to professional practice. Simply make the request by email or telephone.

Retention and destruction

Consultation requests and related exchanges are kept for five (5) years, in accordance with applicable professional practices. After this period, the information is securely deleted.
